FAQs Regarding College-Bound Dependents 

(This benefit is available to all fully-insured employer-group plans and some self-insured plans. It does not apply to Individual Marketplace)

How are children (dependents) covered if they go to college outside of Piedmont’s network area?
Piedmont has additional provider networks called “expanded networks” that are available to dependents attending college outside of Piedmont’s network area. To qualify, please complete the Out-of-Area Dependent Registration Form or CLICK HERE . You can also call Customer Service* for information regarding expanded networks available to dependents who live outside of Piedmont’s network area.

Will my child (dependent) attending college outside of Piedmont’s network area get an ID card with his/her name on it, as well as the name of the provider network he/she may use to receive in-network coverage costs?
Yes, after we receive your completed Out-of-Area Dependent Registration Form, we’ll send a new card to the address on file. It will include the names of the subscriber and the dependent, as well as the name of the provider network to be used by the dependent while living outside of Piedmont’s network area. For additional or replacement cards, login to the Member Portal or CLICK HERE and select “ID card,” or you may contact Customer Service*.  

How do I notify Piedmont that my child (dependent) is attending college outside of Piedmont’s network area? 
You must complete the Out-of-Area Dependent Registration Form or CLICK HERE .  Once completed, mail, deliver or fax as instructed on the form.  

How does an out-of-area dependent or college student identify an in-network doctor?
The network of providers available to an out-of-area dependent is printed on his/her ID card. To find providers within that network, go to pchp.net, select “Provider Search,” scroll down to “Piedmont Community Health Plan Expanded Networks,” and select the network listed on your ID card. There you’ll find a list of providers and their contact information within the expanded network where your child is or will be residing. For more information, call Customer Service. *

How often do I need to update the information on the Out-of-Area Dependent Registration Form?
The form should be updated when the dependent leaves the network area to which he/she is assigned. The update may be made by completing a new form or by contacting Customer Service.*
