Delegated Entities must submit an attestation to Piedmont that demonstrates your organization’s awareness, completion and compliance with Piedmont and CMS federally regulated program requirements.  Please complete and submit the form to fulfill your annual requirements.  Be sure to print a copy of the attestation page for your records.  (Note: The principal of the organization must submit the attestation to Piedmont, not the employees.)

Important Note:  Documentation of annual training and code of conduct distribution is your responsibility.  You are required to maintain records documenting your organization’s compliance with all of these requirements.  Piedmont or CMS may request that you provide documentation of your compliance with these requirements.
