Program of All-Inclusive Care for the Elderly
For many older adults, the thought of leaving home for nursing care can feel overwhelming. PACE, or the Program of All-Inclusive Care for the Elderly, is designed to meet the needs of elderly care.
“PACE provides wraparound healthcare services for older adults that allows them to remain safely in their home after being found eligible for nursing facility care,” said Ashley Shelton, PACE Community Outreach Manager. Eligibility is determined by a state screening for adults 55 and older who live in a service area and qualify for long-term care.
Unlike nursing homes or traditional senior care, PACE is designed to keep participants at home and connected to their community. “All our participants return to their homes, sleep in their own beds, and remain in their communities with family and loved ones, while receiving the support they need,” Shelton shared.
Through an interdisciplinary team approach, PACE covers nearly every aspect of care.
Services include:
- Transportation to appointments and day centers
- Primary care with a 24/7 on-call number
- Physical and occupational therapy
- Home care services
- Social work support
- Meal delivery
- Planned engagement activities
- Medical equipment
Participants can select the supports and services they need to assist them in reaching and maintaining their goals and independence. Each participant is assessed within 30 days of enrollment and reassessed every six months. A care-team approach rounds out a collaborative individualized plan of care.
While PACE is designed to support participants, it also provides relief to caregivers. Five day centers are open Monday through Friday, offering safe environments for participants while loved ones work or attend to other responsibilities.
“PACE acts as the quarterback of our patient’s care,” Shelton said. “We coordinate across care settings, make recommendations, and implement changes right away.”
PACE has served Central Virginia for over 16 years, caring for more than 400 patients. Community members and healthcare providers are encouraged to tour a center or request an educational in-service.
