Laura Campbell on Managing Diabetes and Chronic Disease

Laura Campbell, Manager of Centra’s Comprehensive Care Management Program, is passionate about helping Centra employees and their families take charge of their health and well-being, especially when living with chronic conditions like diabetes.

“Centra’s Comprehensive Care Management Program is offered to all employees and their dependents enrolled in the
health plan,” Campbell explained. “This program offers an ongoing relationship with knowledgeable staff who help assist you through an illness and/or coach you toward wellness.”

The program, which is free, confidential and voluntary, provides support for everything from short-term health questions to long-term, complex disease management. “Someone might need help that can be completed in just a few short calls, or they may need longer-term complex management and anywhere in between,” Campbell said. “You can engage with us at any time you’re ready for assistance.”

Centra’s Comprehensive Care Management Team focuses on empowering members to understand their conditions, set achievable goals and stay engaged in their care. In 2026, the program will expand its offerings to include health coaching and disease management.

"Health coaching is a collaboration between our team and the member to improve overall well-being," Campbell said. "We focus on physical activity, nutrition, sleep, stress management and weight management."

Meanwhile, disease management helps those living with chronic conditions like diabetes, hypertension, hyperlipidemia and low back pain. “Our goal is to improve quality of life, prevent complications and provide education,” Campbell said. “Whether someone needs simple check-ins or weekly support, we tailor our approach to meet each person’s unique needs.”

When a member joins the program, the first step is an assessment to understand their situation and health goals. “We work with individuals to break down their goals into achievable steps that don’t feel overwhelming,” Campbell said.

For those with multiple or complex medical needs, the team provides a team-based approach centered on the individual. “We help offer tips and resources that improve communication, reduce duplication of care and promote preventative care,” she explained. “We’re not a substitute for your provider, but we’re here to empower you to make better choices for your overall health and wellbeing.”

Managing diabetes specifically can feel daunting, especially for Caregivers who spend much of their time focused on
others. “We see Caregivers having a variety of challenges, including prioritizing their own health and finding the time to take care of themselves,” Campbell shared. “Making lifestyle changes and managing the emotional and psychological challenges of living with a chronic condition are also common struggles.”

The team helps by meeting members where they are, physically and emotionally. “We learn about the individual’s understanding of their condition, provide education and help them build confidence in managing their health,” she shared. “We engage members with empathy, motivate them to make positive changes and focus on their whole health, not just their diagnosis.”

For those who might be hesitant to reach out, Laura has one simple message: “Take the time to invest in yourself and your own health and wellness. Our program is here when you’re ready.”

And as Centra recognizes Diabetes Awareness Month, she offers a reminder to all Caregivers: “As Caregivers, we often prioritize others. This month, I encourage you to take a moment to focus on your own health and wellness journey.”
